During a private, comprehensive evaluation, we listen, perform precise testing, answer your questions, and begin problem solving.

Heather received a B.S. degree in Physical Education from Portland State University in 1976.
At PSU, she was honored as the Outstanding Woman Graduate from the department of Physical Education.
Heather graduated with her degree in physical therapy from Pacific University in 1978.
During the initial evaluation, your therapist will take your history, make an assessment, and design an individualized treatment plan that includes goals, frequency, and duration.
Please complete your paperwork before arriving and allow 15 minutes for processing.
If you did not fill it out, please arrive 30 minutes early.
Be sure to bring your insurance cards and physician's order, if necessary.
Your condition will be evaluated at your initial appointment.
Please make every effort to attend therapy sessions as scheduled to receive the most benefit from your treatments.

Medicare - Traditional & Advantage, but Providence only if plan has Out-of-Network Benefits.
PacificSource Health Plans, EBMS, No TMJ coverage, Legacy Health Plan-Call to check benefits.
Please note that insurances accepted are subject to change.
We also have a flat fee price available to patients who are not covered under any insurance.
