When you come in to Eclipse you will be asked to complete intake paperwork before being evaluated by one of our highly trained and professionally licensed physical therapists, if you haven't already downloaded and completed it.
Your evaluating therapist will discuss your case with you, perform a comprehensive examination, and work together with you to develop an individualized plan of care to assist you in reaching your PT goals.
