Elevate Physical Therapy, Inc. was built by Dr. Crider and Dr. Davenport to break the mold of physical therapy. At Elevate Physical Therapy, Inc. our core mission is to enable our patients to obtain a life of independence. For our weekend warriors and older adults this means independence from those nagging joint pains which prevent you from finishing what needs to be done.

For our patients with chronic pain, this means a life independent from pain medication and injections, wheelchairs and recliners. Finally, for our athletes this means independence from braces, taping, ice baths, and overuse injuries. So if fulfillment, enjoyment, and independence is what you seek out of life, then Elevate is your physical therapy provider.

The current state of healthcare is hallmarked by overbearing insurance companies, long waits to see your provider, and expensive procedures and medications which do not provide adequate relief.
We aim to change this climate with our patients, by streamlining your healthcare and wellness.
You will receive one on one care with your physical therapist, giving you higher quality care by focusing solely on you.
Non-treatable pathologies will be recognized faster and referred out to the appropriate provider.

I strive to facilitate positive change with every interaction.
As a physical therapist this comes in three ways.
The first is to make a positive impact in your limitation be it pain, weakness, balance, safety, or range of motion.
This can be done with many types of approaches.
My approach is to listen to you and see if there is a pattern in your daily life.
Typically our lifestyle and habits form our problems.
I then provide modifications to your day with movement, posture, and habits.
Second, I help empower you with knowledge.
When a physical therapist has a contract with an insurance company, the insurance company has the ability to dictate what the PT can do by controlling what services are reimbursed.
As an out of network provider, we customize your care to exactly what you need, while still being able to apply your payments to your benefits.
So how does this save you money?
When you go to an in-network provider you will pay a co-pay (usually between $40-100) then you will receive a bill from your insurance saying that the provider charged you the full amount for the session (usually around $350), but they only allow you to be charged the contracted rate which is less but could still be around $275 for example.
Elevate Physical Therapy, Inc. has decided not to have our practice restricted by insurance companies.
Over the past several years, insurances have limited physical therapists' ability to practice the best way they see fit by refusing or reducing reimbursement for certain services.
In a traditional physical therapy setting, you can expect to be seen multiple times per week regardless of the need for it.
With this in mind and the fact that insurance copay's can average $40 per visit (and being as high as $100) you can actually save money being seen by us.
