One of the most important things that we do at Inglewood Physical Therapy is listen to You, our patient. Once we understand what you want to accomplish, we then structure a personalized program focused on you to achieve your goals.

Our team of highly trained professionals uses the latest healing technologies to restore you to pain-free health, quickly and easily. We thoroughly evaluate & treat all of the contributing root factors related to your issue. This includes, but is not limited to, your work and home stressors, overall body condition, nutrition, genetic & postural habits, emotional connections and patterns that are held in your muscles.

Chris graduated with his Masters degree in Physical Therapy from the University of Michigan and has advanced training in Manual Therapy (OMT) and Orthopedics. He has been a practicing clinician for more than 27 years.

As a courtesy, we will check your insurance benefits after you schedule an appointment with us.
However, if you'd like to know what they are ahead of time, you can call your insurance company and ask the following questions:.
Our self-pay rates are $150 for the initial evaluation (1st appointment) and $112.50 for follow up.

Medicare and Non Medicare patients, your paperwork packet can be found at the clickable files below.
This can be filled out online and printed or you can print a blank copy and fill it out manually.
Either way, please fill it out and bring it with you to your first appointment.
Please arrive 15 minutes early (30 minutes if you are a Medicare patient) to fill out paperwork.
You may also do the paperwork ahead of time by downloading the forms from our website.
If you do this, we still ask that you come in 5 minutes early so that we may get copies of your ID, insurance card, doctor's referral, and collect any co-pays.
