Burhani Physical Therapy and Rehabilitation is an independently owned private out-patient clinic located in Cypress TX since 2006. We are licensed by the state of Texas and registered with the Texas State Board of Physical and Occupational Therapy. We work in close collaboration with you, your physicians, case managers, and insurances in achieving this goal.

A: Once your doctor has prescribed physical therapy, call us.
We will need your name, date of birth, and insurance information, to verify your benefits.
If you are injured at work, and covered under worker's compensation we will need your claim number and case manager's name and contact number.
If you are covered by auto insurance, we will need claim number, claim adjustor's name and contact number.
We will try and set up an appointment as soon as possible, once we verify the benefits.
A: Please come 10-15 minutes before your appointment time to complete paperwork.
