Fill them out and bring them with you to your first appointment.
If your insurance type is not listed, please check with us before filling our the documents to ensure that we accept your insurance carrier.
If you have problems downloading or printing these documents and cannot bring in your completed packet on your first visit, please arrive 15 minutes early to allow time to fill out the documents in our office.
Below, you will see a list of terms that pertain to insurance coverage and payment for health services.
Co-insurance: in indemnity, the monetary amount to be paid by the patient, usually expressed as a percentage of charges.
Co-payment: in managed care, the monetary amount to be paid by the patient, usually expressed in terms of dollars.
Consumer Driven Health Care (CDHC): refers to health plans in which employees have personal health accounts such as a health savings account, medical savings accounts or flexible spending arrangement from which they pay medical expenses directly.
You will provide us with your referral for physical therapy if you have one.
If you don't have a referral, let us know when you schedule your first visit with us (note - a referral is not always needed).
Palpation - touching around the area of the pain/problem.
This is done to check for the presence of tenderness, swelling, soft tissue integrity, tissue temperature, inflammation, etc.
Range of Motion (ROM) - the therapist will move the joint(s) to check for the quality of movement and any restrictions.
