Be sure to arrive approximately 15 minutes prior to your appointment time to provide time to review all forms, photocopy your insurance card and identification card, and have you change into appropriate clothing, if needed.
Based on the injured body part, please select the correct functional assessment, complete the questionnaire then print and bring in your results.
Your first visit at PAPT will be your initial evaluation where you will meet your physical therapist.
They will perform a comprehensive evaluation and discuss your current ailment and identify your goals for physical therapy.
During this appointment, your physical therapist will not only evaluate the body part that is painful but also the surrounding body parts of the injury to determine if other body parts are contributing to your area of complaint.
At the end of your evaluation, your physical therapist will discuss his/her determination of what the problem is and develop a treatment plan that they feel will help you achieve "your" goals.
After the initial evaluation, you will continue to be treated by the same therapist, guaranteeing consistent treatment for each session.
Each follow up session will be approximately one hour to one and an half hours in length, with additional stretches and strengthening exercises added to your individually designed routine periodically.
Please be on time for your appointments in order to obtain full benefit from treatment.
If you are unable to make your appointment, please give us 24 hours notice for rescheduling.
