A Nurse Coordinator, assigned to all wound patients, works closely with the family and the wound physician to monitor progress and serve as a resource to address concerns.
The collaboration with the Primary Care Physician to ensure appropriate follow up care is coordinated upon discharge from Oakridge Nursing & Rehab.
Our interdisciplinary team, Wound Physician, Licensed Dietician, Social Worker, Physical Therapist, Occupational Therapist, Speech Therapist and Nursing staff consistently reviews the individualized plan of care.
