Foundation Hand & Physical Therapy was established in 2015 by Brad and Tonya Cummings, whose mission was to provide high-quality, patient-centered physical therapy in an environment where everyone feels welcome and supported.

This privately owned clinic in Richmond, KY, offers outpatient physical and occupational therapy for a variety of orthopedic injuries and conditions, especially those affecting the upper extremities.
Services
Foundation Hand & Physical Therapy provides one-on-one therapy sessions.
Patients stay with the same therapist throughout their treatment, and our therapists regularly communicate and consult with physicians about treatment plans.
Some of the conditions that we treat include strains and sprains, fractures, joint replacement, pediatric conditions, sports injuries, work-related injuries, and chronic pain.
We also have two full-time certified hand therapists on staff.
These are therapists who have received extra training and education in conditions of the hand, wrist, elbow and shoulder.
With five licensed PTs and a PTA on staff, Foundation offers treatment for a variety of orthopedic injuries for adults and children.
Foundation employs two Hand Therapists who have received specialized training and achieved official certification from the Hand Therapy Certification Commission of the United States and two occupational therapists who are pursuing their certification.
With more than 20 years experience working with employers and their employees, we provide one-on-one hand and physical therapy sessions incorporating advanced therapeutic procedures, research-supported exercise, and patient education in a holistic approach to treatment.

1. Write down any symptoms you've been having and for how long and when they are aggravated (times of the day/with certain activities).
2. Write down key information about your medical history, even if it seems unrelated to the condition for which you are seeing the physical therapist, including any medications you are taking.
3. Bring a list of the names of your physician and other health care professionals that you would like your physical therapist to contact regarding your evaluation and your progress.
